Job summary

Integral Care in Austin, Texas is hiring a part-time Collaborative Care Case Manager (CCCM). You will work with a small team to complete biospsychosocial assessments and person-centered recovery plans, provide skills training, and coordinate cross-system collaboration to help clients meet their goals.

This role requires a Bachelor's in Human/Behavioral/Social Services (Master's preferred), a valid driver's license, and at least one year of experience with intellectual/developmental disabilities.

Qualifications

• Bachelor's degree in Human/Behavioral/Social Services or related field (Master's degree preferred).
• Current and valid driver's license with a clean driving record.
• At least one year of experience working directly with persons with intellectual disability or other developmental disabilities.

Responsibilities

• Deliver therapeutic interventions fostering skills to manage symptoms/crises.
• Provide support/training to the system of support and obtain services for clients.
• Coordinate cross-systems collaboration and develop biospsychosocial assessments and recovery plans.

Education

Bachelor's degree in Human/Behavioral/Social Services or related field
