Job summary

Integral Care is hiring a part-time Collaborative Care Case Manager to conduct biospsychosocial assessments and build person-centered recovery plans in the community. You will provide skills training, complex case management, and collaborate across systems to help clients meet goals.

The role requires driving and flexible hours Monday–Friday. Ideal candidates have a Bachelor's in Human/Behavioral/Social Services or related field; a Master’s is preferred.

Qualifications

• Bachelor's degree in Human/Behavioral/Social Services or related field is required.
• Master's Degree preferred for this role.

Responsibilities

• Conduct biospsychosocial assessments and develop person-centered recovery plans.
• Provide skills training, complex case management, and cross-systems collaboration to help clients meet goals.
• Assist clients with accessing services such as school, medical care, nutrition, financial assistance, transportation, and housing.
• Work under LHPA supervision and maintain trauma-informed practice in a community setting.

Education

Bachelor's degree in Human/Behavioral/Social Services or related field
