Job summary

Texas Panhandle Centers is hiring an Intellectual & Developmental Disabilities Case Manager to deliver trauma-informed care across Amarillo and surrounding areas. The role supports adults with IDD, coordinating services, assessments, and transition planning within a multidisciplinary team.

Qualifications

• Bachelor’s degree required for QIDP credentialing.
• Excellent time management and ability to manage a caseload independently.
• Proficient with Microsoft Word and Outlook; strong writing and verbal skills.

Responsibilities

• Maintain ethical, trauma-informed communications at all times.
• Travel within the Texas Panhandle to serve IDD individuals.
• Assist transitions from facilities back into the community and enroll in Home and Community-based Services.
• Provide ongoing assessments, treatment planning, and documentation per deadlines.
