Job summary

Integral Care in Austin, Texas is hiring a part-time Collaborative Care Case Manager (CCCM). You will work with a small team to complete biospsychosocial assessments and person-centered recovery plans, provide skills training, and coordinate cross-system collaboration to help clients meet their goals.

This role requires a Bachelor's in Human/Behavioral/Social Services (Master's preferred), a valid driver's license, and at least one year of experience with intellectual/developmental disabilities.

Qualifications

Job description

Outpatient Biopsychosocial Interventions (OBI) for people with Intellectual/Developmental Disabilities (IDD) is hiring a part time Collaborative Care Case Manager (CCCM). As CCCM, you will work closely on the small team to complete biospsychosocial assessments and person center recovery plans. You will provide skills training, complex case management, and intensive cross systems collaboration to help clients meet their goals, and improve their health.

Individuals served through OBI will have IDD co-occuring mental health needs such as serve mental illness, substance use disorders and/or chronic health conditions. OBI staff operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile. As a part time CCCM there is flexibility to determine your work days and hours between the Monday through Friday eight to five work week.

As a CCCM, you may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms/crises, provide support/training to the system of support, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance and resources fulfilling any basic need. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of an LHPA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment. Additional job duties may be assigned to meet the needs of the program.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

• Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's Degree preferred.

License:

Required:

• Current and valid driver's license and a clean driving record.

Experience:

Required:

• Has at least one year of experience working directly with persons with intellectual disability or other developmental disabilities.

Preferred:

Hours: Part - Time.

Workdays: Monday-Friday.

Work hours: 8:00 AM- 5:00 PM.

Salary: $24.18 - $25.39, an hour based on education.
