Job summary

Job description

Responsibilities

The position serves as an IDD Intake Coordinator serving all eight counties (Bastrop, Burnet, Lee, Fayette, Gonzales, Caldwell, Guadalupe and Williamson). The coordinator is the point of entry for publicly funded intellectual and developmental disability (IDD) programs, whether provided by a public or private entity. The role includes enrolling eligible individuals into the following Medicaid/GR programs: ICF/IID, Community First Choice (CFC), GR Community Services, and Autism Services. The Intake Coordinator provides services to children and adults with Autism, developmental, or intellectual disabilities based on the Person-Centered Thinking model through assessment of needs, development of service plans, facilitating service linkages, monitoring effectiveness, crisis intervention/prevention support, and advocacy services. Intake duties include completing eligibility screenings for various funding streams (ICF/MR, CFC, Autism and General Revenue), referrals to generic resources, admissions to GR, referrals to ICF/MR and CFC services, autism services, and crisis intake support and intervention. The position requires frequent use of initiative and independent judgment, assertive professional intervention, telehealth and/or in-person contact, flexible hours, and willingness to travel within the counties.

Differential Detail
  • Up to $4.00/hour – Tenure with BTCS
  • $.75/hour – Bilingual
  • $1.00/hour – LCSW-A, LPC-A
  • $.50/hour – LCDC Licensure

Location: Remote and in person (Bastrop, Burnet, Lee, Fayette, Gonzales, Caldwell, Guadalupe or Williamson Counties). Some travel within the 8 counties may be required.

Minimum Qualifications

A bachelor’s degree or higher with a major in social, behavioral, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice. Must have a valid Texas driver’s license and maintain approved driver’s status throughout employment.

Preferred Qualifications
  • At least one year of case management or service coordination experience working with individuals diagnosed with Intellectual or Developmental Disability.
  • Knowledge of appropriate community resources and experience with Medicaid Waiver programs.
  • Familiarity with the SmartCare program.
  • Bilingual.
