Job summary

Model Cities of St. Paul, Inc. is seeking a Housing Case Manager to help individuals and families obtain and maintain safe, stable, and affordable housing. You will provide case management, housing navigation, advocacy, and resource coordination to support long-term self-sufficiency.

The ideal candidate is compassionate, organized, and committed to trauma-informed, culturally responsive services, with experience in housing or human services and strong tenant-rights knowledge.

Qualifications

• Bachelor's or Associate degree in Human Services or related field, or equivalent combination of education and experience.
• 1–3 years of experience in case management, housing services, or related work.
• Strong communication, organization, and relationship-building skills.
• Proficiency with Microsoft Office and data management systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess participant needs and develop individualized housing stability plans.
• Provide ongoing case management, coaching, advocacy, and follow-up.
• Assist participants with housing searches, applications, move-ins, and retention.
• Connect participants with community resources and supportive services.
• Educate participants on tenant rights and housing success strategies.
• Build relationships with landlords, housing providers, and partners.
• Maintain participant records and ensure compliance with program requirements.
