Job summary

The Amherst H. Wilder Foundation in Saint Paul, MN is seeking a Regular Full-Time Case Manager to provide culturally responsive, trauma-informed services and referrals to help clients maintain or stabilize housing.

You will complete daily documentation, interpret provider reports and assessments, conduct in-person home visits, and maintain complete and accurate client records, contributing to a holistic, community-focused approach.

Qualifications

• AA degree in Social Work, Psychology, Sociology, Human Services, or related field is required, with at least two years of relevant work experience.
• BA degree in a related Human Services field preferred, with at least one year of relevant work experience.
• A solid understanding of homelessness, mental health issues, substance abuse, navigation of housing resources (including eviction prevention), domestic violence, and community resources is preferred.
• Candidates must possess a valid Minnesota driver’s license and proof of insurance.
• This job requires Case Managers to conduct in-person home visits.

Responsibilities

• Provide culturally responsive, trauma-informed case management services and referrals to community resources to remove barriers to housing stability.
• Complete daily documentation, interpret reports and assessments from providers, and maintain complete and accurate client records.
• Conduct in-person home visits as part of case management responsibilities.

Education
