Job summary

Wilder seeks a dedicated Case Manager in Saint Paul, MN to deliver trauma-informed, culturally responsive case management and referrals to help clients maintain housing. You will complete daily documentation and maintain accurate client records, with in-person home visits required.

AA degree in Social Work or related field is required, with at least two years of relevant experience; a BA is preferred, with four years experience possible in lieu of the degree.

Qualifications

• AA degree in Social Work, Psychology, Sociology, Human Services, or related field is required, with at least two years of relevant work experience.
• BA degree in a related Human Services field preferred, with at least one year of relevant work experience. Strongly relevant work experience of at least four years may substitute for the BA degree.
• Candidates must possess a valid Minnesota driver’s license and proof of insurance.
• This job requires Case Managers to conduct in-person home visits.

Responsibilities

• Provide trauma-informed, culturally responsive case management and referrals to community resources to help clients maintain housing.
• Complete daily documentation, interpret reports and assessments from providers, and maintain complete and accurate client records.
• Conduct in-person home visits as part of case management.
