Job summary

Brooklyn Community Services is seeking a Case Manager for the Family Shelter to deliver trauma-informed, client-centered case management to families residing in the shelter. You will assess needs, coordinate housing placement, benefits access, and services to support stability and move toward permanent housing.

As part of the Health, Housing and Homeless Solutions division, you will collaborate with shelter staff, DHS CARES, and community partners, maintain accurate records, and attend weekly

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field.
• MSW preferred.
• Minimum one year of case management experience in shelter, housing, or family services.
• Familiarity with NYC housing resources, public benefits, and DHS CARES is a plus.
• Bilingual ability strongly preferred (Spanish or other language).

Responsibilities

• Carry assigned caseload of families; conduct regular meetings to assess needs and progress.
• Develop individualized service plans addressing housing, income, childcare, health.
• Connect families to benefits, employment, legal services, childcare, and mental health supports.
• Assist families in completing housing applications, documentation, and eligibility for subsidy programs.
• Maintain case records in CARES and DHS systems; prepare required reports.
• Attend supervision, case conferences, and team meetings.
