Job summary

StepStone Family & Youth Services seeks a compassionate Foster Care Case Manager to support children and families in foster care. You will provide individualized case management, advocacy, and service coordination across multiple partners to ensure safety, stability, permanency and well-being.

The role requires coordinating with foster parents, families, schools, and community providers, maintaining documentation, and promoting trauma-informed, strengths-based practices for positive outcomes.

Qualifications

• Bachelor's degree in a human services field or nursing.
• Valid driver's license.
• At least two years of case management experience.
• Strong verbal and written communication skills.

Responsibilities

• Respond to referrals and assess clients' needs with the clinical team.
• Complete pre-screening, reports, and case proposals.
• Oversee field staff supporting pre-screening.
• Maintain account info and payer contracts as appropriate.
• Consult with third parties on client benefits and reimbursement.
• Communicate case acceptance and assign clinical teams.
• Develop knowledge of services and community resources.
• Maintain relationships with referral sources and track satisfaction.
• Ensure compliance with policies, HIPAA, and ethics.
• Participate in planning meetings and special projects.
