Job summary

BrightSpring Health Services, in partnership with StepStone Family & Youth Services, seeks a compassionate Foster Care Case Manager in Louisville, KY. You will coordinate individualized case plans, advocate for youth, and collaborate with foster parents, schools, and providers to promote safety, permanency, and well-being.

The ideal candidate has a Bachelor's degree in a human services or nursing field, at least two years of case management experience, and a valid driver's license.

Qualifications

• Bachelor's degree in a human services field or nursing field.
• Valid driver's license.
• Two years of experience with case management or related discipline.
• Ability to communicate effectively verbally and in writing.

Responsibilities

• Receives/responds to incoming calls from referral sources and consults with Clinical Management to determine capability to meet needs.
• Completes pre-screening and pre-admission paperwork, including authorization for payment and coordinating treatment team availability.
• Oversees field staff assisting with pre-screening processes.
• Maintains knowledge of services/resources provided by the Company and community resources.
