Job summary

BrightSpring Health Services is seeking a Foster Care Case Manager to support children in foster care through trauma-informed, strengths-based, family-centered practices. You will coordinate services, maintain regular contact with families, and help ensure safety, stability, and permanency.

The role involves case management, individual and family counseling, and collaboration with treatment teams, schools, and community providers to promote positive outcomes for youth in care.

Qualifications

• Bachelor's degree in social work, sociology, psychology, counseling, or related human services field required.
• Two years' experience working with children and families required; five years with a placing agency may substitute for a graduate degree.

Responsibilities

• Maintain weekly contact with foster families, children, and bio-families and monitor progress.
• Prepare written treatment plans and progress reports; conduct and document case progress.
• Develop individual treatment plans for troubled children and families within 21 days of placement; update quarterly.
