Job summary

The Landing in Houston, TX is seeking a Case Manager 1 for the Phase 1 Drop-In Center. You will provide direct, trauma-informed services to individuals in the earliest stage of engagement, helping with stabilization and basic needs while guiding clients toward ongoing support.

You will build trust, coordinate housing, benefits, health, legal referrals, and transportation, maintain accurate records, and collaborate with internal teams and community partners.

Qualifications

• Bachelor's degree in Social Work, Psychology, or a related field required.
• LMSW/LCSW/LPC or related license/certification preferred.
• Bilingual English/Spanish preferred.
• At least one year of relevant experience with high-risk or vulnerable populations.
• Valid driver's license and reliable transportation required.

Responsibilities

• Engagement: Build rapport and encourage participation in Drop-In Center services.
• Case Management & Stabilization: Provide individualized case management and crisis support.
• Housing & Benefits: Coordinate housing assessments and government document needs.
• Health, Behavioral Health & Legal Support: Facilitate access to counseling and legal referrals.
• Transportation & Practical Support: Provide transportation to critical appointments.
• Documentation: Maintain accurate records in the case management database.
• Program Support: Monitor operations and co-lead life skills classes for Phase 1 clients.
• Collaboration & Outreach: Work with internal teams and community partners.
