Job summary

The Salvation Army USA Western Territory in Seattle, WA seeks a full-time Case Manager for the Bills Bed/Unified Care Team. You will deliver trauma-informed care, perform housing readiness assessments, develop ISP, and provide intensive case management to clients who are experiencing homelessness.

This role requires a Bachelor's degree in social work or human services, 3+ years in related fields, and the ability to work in-person.

Qualifications

Bachelor's degree in social work or human services.
Three plus years of experience in case management, housing navigation, or similar.
Experience working with persons experiencing homelessness in shelter/residential programs.

Responsibilities

Provide comprehensive case management including screening, ISP development, and crisis intervention.
Connect clients to health, mental health, employment, housing, and other community resources.
Monitor progress and adjust service plans as needed.
Assist clients with applications for Social Security or other state assistance.
Maintain professional, ethical interactions with clients, guests, vendors, and colleagues.
Coordinate staff training and mentor colleagues as needed.
