Job summary

The Salvation Army Seattle is seeking a dedicated Case Manager for the Bills Bed/Unified Care Team to provide trauma-informed care and intensive social services to individuals experiencing homelessness. You will conduct housing readiness assessments, develop ISPs, and help clients move toward self-sufficiency within a supportive, client-centered framework.

Responsibilities include comprehensive case management, crisis intervention, resource linkage, and coordination with program staff.

Qualifications

• Bachelor’s degree in social work or human services; 3+ years of case management experience preferred.
• Experience in housing navigation, housing resources, or similar support for people experiencing homelessness, including co-morbidities.
• Experience working with persons experiencing homelessness in shelter/residential programs.

Responsibilities

• Provide comprehensive case management, including screening, assessments, ISPs, and crisis intervention.
• Connect clients to health, mental health, employment, housing, and community resources.
• Maintain client records and report progress in meetings and to supervisors.
• Assist clients with applications for Social Security or state assistance as needed.
• Foster a professional, welcoming environment and coordinate training for staff.
