Job summary

ProJeCt of Easton in Easton, PA is seeking a full-time Case Manager to help people move from a shutoff notice or pantry shortage to a next step they can use. The role connects emergency assistance, housing and utility help, literacy, youth services, and the food pantry with trauma-informed case management.

You will assess needs, build service plans, and coordinate with landlords and partners to unblock problems, while documenting notes for audits and ensuring referrals are followed through.

Qualifications

• Must have an associate degree or equivalent case management experience.
• Follow through on plans, referrals and files.

Responsibilities

• Assess needs and build a service plan and update as life changes.
• Help people apply for rental, heating, and utility assistance.
• Navigate SNAP, Medicaid, LIHEAP, housing, jobs, transportation, and healthcare, then hand off inside ProJeCt.
• Work with landlords, utilities, and partners to unblock a specific problem.
• Support adult learners, families, and Out-of-School Youth when assigned.
• Write case notes the same day and keep funder files audit-ready.

Education

Associate degree in human services, social work, psychology, education, or related field
