Responsibilities

• Maintain confidentiality of client information and obtain consent as required.
• Provide intensive case management focusing on self-sufficiency and housing needs.
• Assist residents in locating and securing housing and provide follow-up support.
• Develop and review case management plans and safety planning with survivors.
• Coordinate referrals to internal and external resources (housing, legal, counseling, mental health).
• Attend case conferences with partner agencies to coordinate services.
