Job summary

Archdiocese of San Antonio seeks a Case Manager to provide comprehensive support for families affected by crisis and hardship. The role includes managing the Mobile Relief Van, conducting home visits at Casa Miguel, and coordinating with partners to deliver housing, food, medical, and legal assistance.

Responsibilities include intake assessments, individualized service plans, and timely referrals to housing programs, benefits, and counseling.

Qualifications

• Education: Bachelor's degree in Social Work, Human Services, or related field preferred; OR 2-4 years of relevant social service experience.
• License and Credentials: Reliable transportation, valid driver license, valid vehicle insurance, must be at least 21, clean driving record.
• Language Skills Preferred: Ability to communicate effectively in Spanish.

Responsibilities

• Conduct client intake and assessment to determine eligibility, immediate needs, and long-term goals.
• Develop individualized service plans addressing housing, employment, documents, and benefits assistance.
• Maintain and report case notes and enter all client assistance to agency data platforms.
• Provide direct case management for Casa Miguel residents and referrals to housing programs, food pantries, medical care, counseling, and legal services.
• Conduct home-visit sessions for Casa Miguel residents, assess barriers to stability and self-sufficiency.
• Develop exit plans outlining goals for transition to independence.
• Assist clients with housing or benefit applications (SAHA, COSA, SNAP, WIC, Medicaid).
• Coordinate with workforce partners and job fairs to help clients achieve employment or training within 90 days.
• Track progress toward goals like housing stability and income.
• Provide transportation assistance for critical appointments.
• Collaborate with internal teams and community partners for wraparound services.
• Prepare weekly and monthly reports on client outcomes, service counts, and community engagement.
• Participate in ongoing professional development and trainings.
• Adhere to safety, confidentiality, and Code of Conduct.

Education
