Responsibilities

• Engage clients enrolled in FBH services and provide referrals to primary health or other FBH services.
• Coordinate appointments using EMR and scheduling platforms with attention to accuracy and privacy.
• Maintain professionalism, reliability, and service orientation with internal/external customers.
• Travel to meet clients as needed and support community outreach.
