Job summary

Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology, or related field.
• Associate degree may be considered with comparable experience.
• Experience with Electronic Health Records (EHR) systems is preferred.
• Strong written and verbal communication skills; ability to work with diverse populations.

Responsibilities

• Screen for psychosocial needs and Social Determinants of Health (SDoH).
• Assess results and connect patients to appropriate resources.
• Develop and maintain referral networks; navigate community services.
• Coordinate care and ensure access to recommended support.
• Assist with intake paperwork and ensure documentation is added to the chart.
• Maintain patient privacy and confidentiality; reduce no-show rates with reminders.
