Job summary

Crossroads Health is seeking a Patient Navigator-Behavioral Health to serve as the first point of contact for clients, providing trauma-informed services by phone and in-person. The role supports intake, documentation, and coordination of care within a collaborative team.

The Navigator will screen clients to identify service needs, engage with treatment providers, and help families access appropriate behavioral health and recovery services, emphasizing compassionate, non-discriminatory care.

Qualifications

• Associate’s Degree in Human Services or related field required.
• Licensed in the State of Ohio with LPC/LSW/MFT (preferred).
• Willing to acquire QMHS within 15 days of hire.
• Prior experience in medical office setting with substantial patient interaction (preferred).

Responsibilities

• Provide trauma informed services to clients primarily on the phone and in the office.
• Assist clients in completing initial required documentation.
• Providing trauma informed individual screening, triage and care coordination to clients and their family system in an office setting as they present for walk-in or call into the intake line.
• Assisting in communication amongst treatment providers and with clients to ensure strong engagement and access to care.
