Job summary

Crossroads Health in Lake County, Ohio, is seeking a Patient Navigator-Behavioral Health to greet clients, screen for services, and connect them with care in a trauma-informed approach.

You will conduct initial intake, assist with documentation, provide triage and care coordination, and support communication among treatment providers and clients to ensure engagement and access to services.

Qualifications

• Associate's Degree in Human Services or related field required.
• Bachelor's degree in social work or related field preferred.
• Licensed in the State of Ohio with LPC/LSW/MFT preferred.
• Willing to acquire QMHS within 15 days of hire.
• Prior experience in medical office setting with substantial patient interaction preferred.

Responsibilities

• Provide trauma informed services to clients primarily on the phone and in the office.
• Assist clients in completing initial required documentation.
• Providing trauma informed screening, triage and care coordination to clients and their family system in an office setting.
• Assisting in communication amongst treatment providers and with clients to ensure engagement and access to care.
