Qualifications

• Bachelor’s degree in social work or related field.
• 1 year experience working with individuals with mental health, substance use, and/or co-occurring needs preferred.
• Meet all WCS and contractor requirements for criminal and caregiver background checks, driver’s record, and drug screen.
• Valid driver’s license, automobile, and sufficient insurance to meet agency requirements.

Responsibilities

• Build trusting, respectful relationships with individuals.
• Utilize a trauma-informed approach when coordinating supports.
• Collaborate with individuals, natural supports, and providers to develop recovery plans.
• Serve as liaison with therapists, healthcare professionals, and case managers for coordinated care.
• Monitor progress toward goals; provide coaching, psychosocial rehabilitation, and symptom management.
• Maintain regular communication via in-person visits, calls, and follow-ups.
