Qualifications

• Pass background checks, driver’s license, and required screenings.

Responsibilities

• Build relationships and provide supportive connections for clients.
• Coordinate care with families, providers, and recovery teams to develop personalized plans.
• Connect clients to community resources and complete referrals.
• Collaborate with therapists, case managers, and partners to ensure coordinated care.
• Monitor progress toward recovery goals and provide coaching and psychosocial supports.
• Document services timely and accurately per regulations and policies.
