Job summary

Pinnacle Family Services, part of Health Connect America, seeks an Integrated Care Coordinator to connect clients with providers and community resources, ensuring comprehensive behavioral and physical health support. You will drive care coordination hands-on, building strong relationships with families and care teams in North Carolina.

The role emphasizes patient-centered planning, collaboration with integrated care staff, and ensuring timely access to services across multiple settings.

Qualifications

• Bachelor’s Degree in Human Services required, with at least two years of experience working with youth or families, or four years in a non‑Human Services field with youth/family experience.
• Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis.

Responsibilities

• Assess client needs and develop individualized, person-centered care plans.
• Coordinate services across treatment teams, primary care, and community resources.
• Facilitate follow-up after hospitalizations or life transitions to support smooth care.
• Monitor progress, track HEDIS gaps, and verify enrollment/payor status for access to services.
• Provide health education and crisis support to clients and families as needed.
• Document care coordination and interactions in accordance with standards.
• Support clinic operations including scheduling, vitals, and chart documentation.
• Collaborate with nurses for integrated care and participate in outreach to promote enrollment.
