Survivors of Torture, International is seeking a Behavioral Health Navigator to improve access to behavioral health and recovery services for individuals affected by crime, violence, and trauma in San Diego. You will provide outreach, screening, care coordination, and system navigation, acting as a central contact for participants and partner agencies.

Formación

• Bachelor's degree in social work, psychology, public health, or related field, or equivalent experience.
• Minimum two years of experience in behavioral health, case management, healthcare navigation, or community services.
• Experience with trauma-informed and culturally responsive practices.
• Experience with diverse and underserved populations; strong communication and organization.

Responsabilidades

• Conduct participant intake, behavioral health screenings, and needs assessments.
• Coordinate referrals and warm handoffs to behavioral health, healthcare, victim services, housing, and community resources.
• Develop individualized service plans with participants and providers.
• Monitor participant engagement and follow-up to reduce barriers to care.
• Maintain timely, accurate, and confidential documentation; collect data for grant measures.
