Job summary

Emergence Health Network is seeking a qualified professional to coordinate services for clients under Medicaid Waiver or non-waiver programs. You will monitor care, complete monthly encounters, and ensure timely, compliant documentation while collaborating with interdisciplinary teams.

The role requires a Bachelor's degree in a social/human services field and at least one year of community-based experience, with preferred knowledge of trauma-informed care and behavioral therapies including DBT

Qualifications

• Bachelor's Degree in social/behavioral or human service field plus 1 year of community-based experience.
• Experience with trauma-informed services and behavioral therapies including DBT and motivational approaches preferred.

Responsibilities

• Provide service coordination to Medicaid Waiver or non-waiver clients, including monitoring and oversight of care.
• Complete monthly encounters with all clients on caseload and maintain documentation to meet targets and ensure timely submissions.
• Apply rules and regulations for Medicaid waiver or non-waiver programs and ensure accurate documentation.
• Attend and facilitate client service planning meetings with LARs and interdisciplinary teams.
• Maintain open communication on caseload, attend team meetings, and manage time to fulfill duties.
