Job summary

LifePath Systems is seeking a Care Coordinator to perform care coordination and outpatient behavioral health services across the LifePath system. You will maintain a caseload, book referrals, and work with partners to ensure timely access to care.

The role requires trauma-informed, person-centered practice, ability to de-escalate crises, and adherence to trainings and policies. Bilingual English/Spanish is preferred to support diverse clients.

Qualifications

• Bachelors degree in psychology, social work, or related field from a four-year college or university.
• CCBHC knowledge and experience required.
• Experience in Texas Recovery and Resiliency (TRR) required.
• Knowledge of Trauma-Informed Care Practices required.
• Must have reliable transportation and a valid driver's license.

Responsibilities

• Provide care coordination and outpatient behavioral health services to individuals across LifePath system.
• Maintain a caseload and coordinate services with community partners to ensure timely access to care.
• Identify needs, reduce barriers to care, and ensure gaps in care are reconciled.
• Assess and de-escalate behavioral health crises in a trauma-informed, person-centered approach.
• Adhere to policies and complete required trainings on time.
