Job summary

LifePath Systems in Plano, TX is seeking a Case Manager to support individuals with behavioral health needs. You will gather information, establish eligibility for community resources, and travel to homes, schools, and community settings.

Responsibilities include case management, skills training, medication training and crisis interventions, documented in the electronic health record with a focus on trauma-informed and person-centered care.

Qualifications

• Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
• At least two years of experience in working with the community, preferred (i.e., social services, recovery, customer service, sales).
• Bilingual (Spanish/English) preferred.
• Must have reliable transportation and a valid driver license.

Responsibilities

• Provide case management, skills training, medication training and support, crisis interventions and psychosocial rehabilitation services.
• Engagement with individuals served and documentation in the electronic health record.
• Frequent travel to individuals' homes, schools, and community settings.
• Work outside of normal business hours to meet program needs.
