Job summary

Pillars Community Health, LLC is seeking a Care Coordinator to link clinical, behavioral health, and social services for children and families under the Pathways to Success program. The role supports home and community-based interventions, coordinates with care teams, and maintains essential documentation.

The position requires a Bachelor's degree or MHP designation, experience with diverse populations, and bilingual fluency in English/Spanish.

Qualifications

• Bachelor's degree in Human or Health Services or Health Education, or MHP designation.
• Experience with diverse child and family populations with intensive behavioral health needs is preferred.
• Experience completing clinical documentation utilizing SMART goals is required.
• Case management experience in home or community settings is a plus.

Responsibilities

• Coordinate care in High-Fidelity Wraparound or Intensive Care Coordination service delivery.
• Meet training and certification requirements of the HF Wraparound model.
• Maintain documentation to meet organizational requirements.
• Facilitate Child and Family Team Meetings as needed.
• Educate participants and families about the Wrap Around Process and Pathways to Success.
• Develop care plans and gather information for IM+CANS and related plans.
• Document care plans and vital information in the EHR after contacts.
• Lead care planning to specify resources and set client-specific goals.
