Job summary

Job description

• Provide trauma-informed care management services to children, adolescents, and families who have experienced or witnessed trauma
• Coordinate community resources and support services
• Monitor treatment progress
• Provide education, supportive counseling, and crisis intervention
• Advocate for the needs of youth and families
• Provide individual and group mental health and substance use disorder case management services
• Refer, advocate, and link clients and families to additional services
• Assist clients with developing daily living, coping skills, and natural support systems
• Transport clients in company vehicles to necessary appointments as needed
• Develop, implement, evaluate, and update treatment plans with client and family input
• Conduct clinical behavioral health assessments
• Enter timely service documentation into the Electronic Medical Record
• Liaise and communicate collaboratively with local law enforcement and stakeholders
• Complete required agency trainings and maintain productivity expectations
• Perform other duties as assigned

Requirements

• Bachelor's degree required
• Good organizational, time management, written and verbal communication skills
• Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries
• Community-based program case managers must have a valid FL driver's license
• Community-based program case managers who take crisis calls and/or home visits must have a valid FL driver's license with a good driving record and current auto insurance
• Competency with electronic medical records, computer systems, and documentation
• Knowledge and understanding of individuals with co-occurring issues, their families, and family systems
• Additional training and/or certification may be required based on specific program guidelines
