Centerstone is seeking a full-time Case Manager in Bradenton/Arcadia to provide trauma-informed care management for children, adolescents and families. You will coordinate resources, monitor progress, educate families, and advocate for youth needs while collaborating with healthcare providers and community partners.

This role provides trauma-informed care management services to children, adolescents, and families who have experienced or witnessed trauma. Responsibilities include coordinating community resources and support services, monitoring treatment progress, providing education and crisis intervention, and advocating for the needs of youth and families. Working collaboratively with healthcare providers, community organizations, and support networks, this position plays a critical role in promoting positive outcomes, enhancing access to care, and supporting the long-term well-being of vulnerable populations. Centerstone's Trauma Services team is looking for a full-time Case Manager in both Bradenton and Arcadia locations.
Effectively communicate via written, verbal, in person and virtual methods.
Knowledge and understanding of the characteristics of individuals with co-occurring issues, their families, and family systems.
Good organizational and time management skills.
Competency with electronic medical records, computer systems, and documentation.
Strong interpersonal communication skills in order to collaborate with team members and maintain professionalism.
Accountable and self-directed with daily job duties.
Knowledge of community resources or ability to learn quickly as well as good organizational, time management and communication skills.
Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries.
Community-based program case managers must have a valid FL driver’s license.
Education Level Bachelor's degree required.

Years of Experience
Certification/Licensure Community-based program case managers, who either take crisis calls and/or home visits, must have a valid FL driver’s license with a good driving record and current auto insurance.
