Responsibilities

• Provide trauma-informed clinical and case management services.
• Conduct initial assessments and brief intervention, stabilization, referrals, and follow-up.
• Coordinate and track service records with excellent note taking.
• Conduct comprehensive assessments and provide DSM-5 diagnoses when appropriate.
• Provide psychotherapy for individuals, couples, families, and groups.
• Develop individualized treatment plans reflecting diagnoses and goals.
• Coordinate with internal programs and external providers with signed releases.
• Provide referrals and assist clients in accessing community resources.
• Collaborate with community partners as needed.
• Conduct home or community-based visits when required.
• Respond to urgent requests and occasional after-hours support.
• Participate in CQI activities and track grant deliverables.
• Maintain weekly caseload of billable sessions.
• Accommodate walk-in clients when possible.

Job description

The Clinical Case Manager provides trauma-informed clinical and case management services to individuals and families impacted by mental health challenges.

Duties and responsibilities

• Conduct timely initial assessments and provide brief intervention, stabilization, referral, and follow-up.
• Coordinate and exhibit excellent note taking and service record tracking.
• Conduct comprehensive clinical assessments and provide DSM-5 diagnoses when appropriate.
• Provide psychotherapy interventions for individuals, couples, families, and groups.
• Develop individualized treatment plans reflecting diagnosis, client needs, and treatment goals.
• Provide trauma-informed, culturally responsive care for clients experiencing mental health challenges, co-occurring disorders, or behavioral concerns.
• Coordinate care with internal programs and external providers, including mental health, employment, medical, and social service agencies, when appropriate and with signed releases of information.
• Provide referrals and assist clients in accessing community resources, if needed.
• Collaborate with community partners when necessary
• Provide home or community-based visits to individuals and families, if needed.
• Respond to urgent community requests and situations, including occasional after-hours support when necessary.
• Participate in CQI activities (Program goals setting, measurement selection, data collection, impacts tracking, quarterly report on CQI)
• Tracks grant deliverable for related fundings
• Maintain a weekly caseload of billable sessions as determined by program expectations.
• Accommodate walk-in clients when possible.
• Create and deliver community education, training, or presentations.
• Maintain timely and accurate documentation in accordance with agency policies and professional standards in a timely manner.
• Process and respond to medical record requests, ensuring compliance with confidentiality laws and agency procedures.
• Attend staff meetings, clinical and case management consultations, and training as required.
• Participate in supervision and professional development activities.
• Maintain professional and ethical standards consistent with licensing and agency policies.

Working conditions

In performing the duties of this position, the employee must have sufficient mobility to accomplish essential job functions. Duties may require bending, stooping, stretching, and occasional lifting. Fine motor skills are required for typing, documentation, and report writing. This position involves a combination of office-based work and community-based services, including visits to client homes or care facilities when appropriate. Work conditions may occasionally include evening or weekend hours, community outreach activities, and working with individuals experiencing emotional distress or crisis situations.
