Qualifications

  • Associate’s or Bachelor’s degree preferred in psychology, human services or related field.
  • Current CPR and First Aid certification or obtained promptly after hire.
  • Experience in case management or behavioral health is preferred.

Responsibilities

  • Provide leadership, supervision, and support to Case Managers to ensure high-quality, client-centered care coordination services.
  • Oversee daily case management operations, including caseload distribution and documentation compliance.
  • Support staff development through coaching, training, and performance evaluations.
  • Assist with recruitment, onboarding, scheduling, retention and coaching as needed.
  • Monitor departmental performance and implement initiatives to enhance client outcomes and operational effectiveness.
  • Collaborate with clinical, medical, admissions, and leadership teams for coordinated service delivery.
  • Ensure timely discharge planning, aftercare coordination, and community resource connections.
  • Maintain compliance with policies, 42 CFR Part 2, HIPAA, licensing and accreditation standards.

Position Summary:

The Client Care Manager is responsible for overseeing the Case Management department and ensuring the delivery of high-quality client-centered care coordination services. This position provides direct supervision, guidance, and support to Case Managers while promoting positive treatment outcomes, continuity of care, and compliance with organizational, regulatory, and accreditation standards.

The Client Care Manager serves as a liaison between clinical, medical, and support departments to ensure clients receive comprehensive services that address treatment, recovery, discharge planning, and community resource needs. This role supports staff development, departmental performance, and program effectiveness.

Essential Duties and Responsibilities
  • Provide leadership, supervision, and support to Case Managers to ensure the delivery of high-quality, client-centered care coordination services
  • Oversee daily case management operations, including caseload distribution, workflow management, documentation compliance and continuity of care
  • Support staff development through coaching, training, performance evaluations, and ongoing professional guidance
  • Assist with recruitment, onboarding, scheduling, retention and coaching as needed
  • Monitor departmental performance, identify opportunities for improvement, and implement initiatives that enhance client outcomes and operational effectiveness
  • Collaborate with clinical, medical, admissions, and leadership teams to ensure coordinated service delivery and successful treatment transitions
  • Ensure timely and effective discharge planning, aftercare coordination, and connection to community resources
  • Maintain compliance with organizational policies and procedures, 42 CFR Part 2, HIPAA, licensing requirements, accreditation standards and other applicable regulations
  • Serve as a resource for staff in managing complex client situations, resolving barriers to care, and addressing service concerns
  • Perform other duties as assigned
Requirements
Qualifications
Education
  • Associate’s degree in Psychology, Human Services, Social Work, Behavioral Health, Sociology or a related field preferred
  • Bachelor’s degree in Psychology, Human Services, Social Work, Behavioral Health, Sociology or a related field preferred
  • Relevant combination of education and experience may be considered
  • Current CPR and First Aid certification required or obtained within the designated timeframe after hire
Experience
  • Minimum three (3) years of experience in case management, behavioral health, substance abuse treatment, social services, healthcare, or a related field preferred
  • Experience working with individuals affected by substance use disorders and co-occurring mental health disorders preferred
  • Knowledge of community resources, recovery supports, and referral processes preferred
  • Strong interpersonal and relationship-building skills
  • Excellent communication and organizational abilities
  • Ability to manage multiple priorities and maintain accurate documentation
  • Proficiency with electronic health records and Microsoft Office applications preferred
  • Ability to work effectively as part of a multidisciplinary team
  • Ability to maintain confidentiality, professional boundaries, and ethical standards
  • Valid driver's license and acceptable driving record preferred
