Job description

  • Conduct intake, outreach, and clinical case management services through a short-term case management model
  • Screen for income supports, benefits, and entitlements and advocate on behalf of program participants
  • Receive case assignments, establish service plans, review case progress, and determine case closure through the assigned program supervisor and according to funder requirements
  • Provide counseling addressing motivational and psychosocial issues in individual or group contexts when needed
  • Provide advocacy, crisis intervention, counseling, and problem solving to support or restore participants’ self-sufficiency
  • Refer clients externally based on client needs or program capacity
  • Collaborate with internal and external program staff to provide services under program agreements
  • Develop, implement, oversee, and follow up on treatment plans to ensure goals and deadlines are met within funder reporting cycles
  • Develop, monitor, and enhance professional relationships with social service, health, and governmental agencies
  • Participate in supervision meetings, team meetings, training sessions, conferences, and program and operational planning
  • Establish and maintain professional and therapeutic relationships consistent with professional standards, confidentiality laws, and current clinical research
  • Perform other duties as assigned
Requirements
  • BA/BSW and 3 years of experience working in a trauma-informed, behavioral, and/or public health setting, or a Master’s degree in social work, public health, counseling, clinical psychology, or related field
  • LPC/LSW licensure preferred
  • Bilingual proficiency in a language other than English, such as Spanish, French, Arabic, or Portuguese, preferred
  • Culturally responsive communication skills
  • Ability to communicate effectively, both verbally and in writing
  • Ability to sit, reach with arms, and work with hands to operate computers or equipment
  • Ability to stand, walk, climb stairs, and may need to kneel or crouch to access workspace equipment
  • May occasionally need to lift or move items up to 10 pounds
