Centerstone in Florida is seeking a full-time Case Manager to provide trauma-informed care management to children, adolescents and families who have experienced trauma. You will coordinate community resources, monitor progress and advocate for clients within a collaborative network of providers.
You will develop and update treatment plans, conduct mental health assessments, document in EMR, and support families through education, crisis intervention and linkage to services.

This role provides trauma-informed care management services to children, adolescents, and families who have experienced or witnessed trauma. Responsibilities include coordinating community resources and support services, monitoring treatment progress, providing education and crisis intervention, and advocating for the needs of youth and families. Working collaboratively with healthcare providers, community organizations, and support networks, this position plays a critical role in promoting positive outcomes, enhancing access to care, and supporting the long-term well-being of vulnerable populations.
Centerstone's Trauma Services team is looking for a full-time Case Manager in both Bradenton and Arcadia locations.
Bachelor's degree required. Good organizational, time management, written and verbal communication skills. Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries.
Community-based program case managers, who either take crisis calls and/or home visits, must have a valid FL driver’s license with a good driving record and current auto insurance.
