Job summary

UC Health in Cincinnati, OH is seeking a Heart/Adult Transplant Coordinator RN to manage and coordinate all aspects of care for solid organ transplant patients and living donor candidates across the continuum—from referral and evaluation through waitlist management, transplant, and post-transplant follow-up.

The role requires Ohio RN licensure with transplant nursing experience preferred, and involves collaboration with a multidisciplinary team to ensure safe, compliant, and patient-centered

Qualifications

• Minimum required: Associate degree. Preferred: Bachelor's Degree.
• Current RN license in the State of Ohio. ACLS preferred.
• A minimum of three to five years of clinical nursing experience required, with transplant experience preferred.

Responsibilities

• Manage patient care across the transplant continuum (evaluation, waitlist, inpatient, and post-transplant).
• Coordinate weekly transplant activities and communicate with multidisciplinary teams.
• Perform clinical assessments, review tests, and elevate abnormal findings to the provider.
• Monitor complications, support ongoing medical management, and reinforce adherence to treatment.
• Maintain documentation in EMR and transplant databases to ensure regulatory readiness.
