Job summary

UC Health in Cincinnati is seeking a Post-Transplant Coordinator to support the transplant program. You will monitor patient status, coordinate care across the transplant continuum, and educate patients and families on meds and adherence.

The role requires an Ohio RN license, 3–5 years of clinical nursing experience, and ACLS preferred. You will work with a multidisciplinary team to ensure optimal graft outcomes and patient safety.

Qualifications

• Registered nurse with Ohio license required.
• 3–5 years of clinical nursing experience; transplant, nephrology, hepatology, critical care, or complex med-surgical nursing preferred.
• ACLS certification preferred.

Responsibilities

• Monitor for rejection, infection, and complications via labs, imaging, and visits.
• Coordinate transplant care across evaluation, waitlist, inpatient, and post-transplant stages.
• Educate patients and caregivers on medications, infection prevention, and self-management.
• Maintain regulatory documentation for adverse events, graft outcomes, and data integrity.
• Collaborate with multidisciplinary teams and participate in quality initiatives.
