Job summary

UC San Diego Health is seeking a Pre-Heart transplant coordinator (multi-site) to manage evaluation and waitlist processes, educate patients, and ensure pre-transplant requirements are completed across Hillcrest, La Jolla, and East Campus.

The role travels to multiple sites for education and coordination, with a focus on regulatory compliance (UNOS, CMS) and collaboration with a multidisciplinary team. RN licensure in California, BART/BLS, and transplant experience preferred.

Qualifications

• Graduate from an accredited school of Nursing.
• Registered Nurse licensed in the state of California.
• Current BART or BLS at time of hire with commitment to obtain BART within 6 months.
• Minimum of one year experience as a pre-transplant coordinator with direct care of pre-transplant patients.
• Experience coordinating multidisciplinary care across inpatient and outpatient settings.
• Familiarity with UNOS/OPTN guidelines and CMS transplant regulations.
• Excellent interpersonal and teamwork skills.

Responsibilities

• Coordinate heart transplant care across multiple campuses.
• Facilitate outside hospital transfers once referrals are accepted.
• Provide clinical oversight and escalate patient changes or care gaps.
• Facilitate timely diagnostic testing and treatment plans.
• Communicate effectively with the multidisciplinary team and with patients and families.
• Serve as liaison between patients, families and the transplant team.
• Participate in rounds and case conferences.
• Navigate Epic and Phoenix to access and update patient charts.
