Job summary

Cardea Health in Oakland is seeking a Program Manager of Transitional Housing to oversee Natalie’s Place and related services. You will supervise clinical and non-clinical staff, coordinate facilities, and work with nursing to move clients from admission to discharge efficiently.

The role requires strong leadership, communication, and problem-solving as you ensure quality care in a trauma-informed, person-centered setting for individuals experiencing homelessness across Alameda County.

Qualifications

• Bachelor’s degree or equivalent professional experience.
• Excellent communication skills with diverse teams.
• Proven leadership and project coordination abilities.
• Strong time management and organizational skills.
• Experience serving homeless populations is a plus.

Responsibilities

• Supervise and direct personnel and program activities for Transitional Housing programs.
• Collaborate with nursing and operations to manage facilities and services.
• Oversee case management, housekeeping, caregiving, food service, transport.
• Support data reporting and program performance assessment.
• Facilitate client flow from admission to discharge and housing connections.
