Job summary

Cardea Health in Oakland is seeking a Program Manager of Transitional Housing to oversee Natalie’s Place and related services. You will supervise clinical and non-clinical staff, coordinate facilities, and work with nursing to move clients from admission to discharge efficiently.

The role requires strong leadership, communication, and problem-solving as you ensure quality care in a trauma-informed, person-centered setting for individuals experiencing homelessness across Alameda County.

Qualifications

  • Bachelor’s degree or equivalent professional experience.
  • Excellent communication skills with diverse teams.
  • Proven leadership and project coordination abilities.
  • Strong time management and organizational skills.
  • Experience serving homeless populations is a plus.

Responsibilities

  • Supervise and direct personnel and program activities for Transitional Housing programs.

Scheduled workdays may vary, and work schedules will generally be communicated in advance. Applicants must be available to work overnight shifts and have weekend availability.Primary Work Location: MultisiteMultisite staff work across several program locations throughout Alameda County, providing care and support wherever it is most needed. These sites offer a range of services, including post-hospitalization medically supported shelter, medical respite care, skilled nursing, and caregiving support.Job Summary:The Program Manager of Transitional Housing will supervise and direct personnel and program activities for Cardea Health Transitional Housing Programs, including Natalie’s Place, a recuperative care program serving people who have experienced homelessness. The Program Manager is accountable for assisting with managing facilities, clinical personnel, housekeeping and caregiving functions, food service, transportation, and housing navigation. In addition, the Program Manager will work with the nursing team to manage the flow of clients through the program, from admission to discharge.Job Duties:Assist the Program Director of Transitional Housing and Director of Nursing for Transitional Housing with leading a team of dedicated clinical and non-clinical professionalsUnder the direction of the Program Director, oversee facilities and operations at a site of unitsSupport the Program Director in manage personnel and operations related to:Clinical staffCase managementHousekeepingCaregivingFood serviceTransportationHousing navigationTroubleshoot issues as indicatedEscalate challenges when appropriateManage client experience in the program from referral to dischargeAssist with data reporting as required for assessment of program performanceMaximize program efficacy in securing service connections and permanent housing for clientsEnsure that clients move through program in accordance with program parametersFill all beds promptlyBasic Qualifications:Excellent communication skills, both written and verbal, with the ability to interact professionally with diverse team members and clientsStrong leadership skills, including team buildingExcellent organizational skills and attention to detailExcellent critical thinking and creative problem-solving skillsExcellent time management skills with a proven ability to meet deadlinesAbility to function well in a high-paced and at times stressful environmentDisplay resilience and resourcefulness in tackling tasks and finding solutionsMust provide proof of COVID vaccination or willing to be vaccinatedHave a passion for serving marginalized populationsProficiency in Microsoft Office Suite is requiredDiscretion and confidentiality in handling sensitive informationProfessionalism, integrity, and a proactive approach to problem-solvingMinimum Required Skills & Qualifications:Bachelor’s degree or equivalent professional experienceAt least two years of related experience requiredProven experience as a team leader and operations professionalExperience working with people experiencing homelessness preferredExperience working in healthcare preferredPhysical Requirements:Occasional sitting or stationary workFrequent standing, walking, kneeling, stooping, and ascending stairsOccasional lifting and carrying up to 20 lbsFrequent pushing and pullingFrequent reaching high, low, and levelWork Environment:Cardea Health program locations operate within supportive housing, interim housing, medical respite, and community-based environments serving individuals experiencing homelessness and other vulnerable populations.Employees may interact with individuals experiencing:Cardea Health is committed to trauma-informed, person-centered, and harm-reduction-oriented service delivery.
