Job summary

Cardea Health in Oakland is seeking a Program Manager of Transitional Housing to oversee Natalie’s Place and related services. You will supervise clinical and non-clinical staff, coordinate facilities, and work with nursing to move clients from admission to discharge efficiently.

The role requires strong leadership, communication, and problem-solving as you ensure quality care in a trauma-informed, person-centered setting for individuals experiencing homelessness across Alameda County.

These sites offer a range of services, including post-hospitalization medically supported shelter, medical respite care, skilled nursing, and caregiving support.Job Summary:The Program Manager of Transitional Housing will supervise and direct personnel and program activities for Cardea Health Transitional Housing Programs, including Natalie’s Place, a recuperative care program serving people who have experienced homelessness. The Program Manager is accountable for assisting with managing facilities, clinical personnel, housekeeping and caregiving functions, food service, transportation, and housing navigation.
