Job summary

Owensboro Health Regional Hospital in Owensboro, KY is seeking a registered nurse to provide direct patient care in the Transitional Care department on weekends. The position is full-time at 0.9 FTE (72 hours per pay period) with a 7A-7P schedule.

The nurse will coordinate with the multidisciplinary team, implement care plans, monitor patient conditions and support discharge planning with patients and families.

Qualifications

• Associate's degree or Diploma in Nursing required upon hire; Bachelor's degree in Nursing required within 5 years.
• RN license in Kentucky required upon hire.
• BLS certification required within 45 days of employment.

Responsibilities

• Provides direct nursing care according to policies and protocols.
• Serves as primary coordinator of all disciplines for well-coordinated patient care.
• Assesses discharge planning needs with patient, family and care team.
• Educates patient and family based on age, culture and learning willingness.
• Performs duties in accordance with the Nurse Practice Act.
