Job summary

Owensboro Health Regional Hospital is seeking an experienced Case Manager (RN) to facilitate cost-effective, quality patient outcomes and transitions by determining the appropriate level of care and providing guidance, education, and support to assigned patients. Qualifications include a Bachelor’s degree or higher in Nursing and being licensed as an RN in Kentucky.

Experience level is not required, and the role is full-time with 0.9 FTE status in Case Management at Owensboro Health Regional

Responsibilities

• Assess patient charts and consider medical, financial, emotional, and support needs.
• Review clinical documentation for medical necessity and appropriate care level.
• Provide clinical information to payers within required timeframes.
• Collaborate with physicians and leaders when hospitalization or discharge delays occur.
• Deliver regulatory letters to patients as needed.
• Work with interdisciplinary team to support care and resource information.
• Assist patients/families with referrals and provide education and resources.
• Educate staff and trainees on case management processes.
