Responsibilities

• Contributes to achievement of department goals and adheres to policies and safety standards.
• Collaborates with the health care team to coordinate plan of care.
• Communicates with care team and family about goals, discharge planning and payer concerns.
• Performs comprehensive pre/post admission assessments to identify patient needs.
• Maintains knowledge of discharge planning requirements and Medicare/Medicaid reimbursement methods.
• Participates in quality improvement and program evaluation activities.
