Job summary

MedStar Health’s Washington Hospital Center seeks aRegistered Nurse in Case Management to apply clinical expertise to care coordination and discharge planning. You will facilitate quality, cost-effective, patient-centered care from pre-admission through post-discharge, ensuring outcomes and resource optimization across acute and post-hospital phases.

The role requires ADN with BSN preferred, 2–3 years of clinical experience and DC/MD licensure eligibility with CCM preferred.

Qualifications

Responsibilities

• Contributes to department goals and adheres to policies and safety standards.
• Collaborates with multidisciplinary team to develop and coordinate the plan of care.
• Communicates with patient, family, and care team to address goals and issues.
• Develops care plans across the continuum and documents outcomes.
• Manages a caseload and identifies needed resources for care plans.
• Participates in performance and service improvement activities.
