Job summary

The GEO Group, Inc. is seeking a transition case manager to provide services to inmates within one year of release, following the Continuum of Care framework and using GEOtrack to document progress.

You will coordinate transition plans, connect to community resources, and conduct individual and group cognitive behavioral sessions, contributing to successful reintegration and reducing recidivism.

Qualifications

• Must be able to prepare written reports and communicate effectively.
• Ability to work with a team of professionals and implement strategies.
• Must be flexible and empathetic to needs of staff and inmates.

Responsibilities

• Participates in training for designated responsibilities.
• Uses GEOtrack to record notes and progress.
• Develops transition plans with inmates and links to community resources.
• Conducts individual and group cognitive behavioral treatment sessions as needed.
• Establishes relationships with community providers to support reintegration.
• Communicates with administration, staff, inmates, visitors, and the public with tact and fairness.
