Job summary

Canopy Roots is partnering with the Metropolitan Council to operate a Community Response Team for Metro Transit's buses, trains, and facilities. The role of Metro Transit Support Team Advocate involves short-term follow-up, case management, and resource referrals to behavioral health, housing, and social services within a trauma-informed framework.

The position is full-time in the Twin Cities metro area with a 40-hour workweek, primarily in St. Paul.

Qualifications

• Associate's degree in social work, psychology, or related field OR 3 years of direct experience.
• Must meet Minnesota state requirements for Behavioral Health Practitioners under MN Statutes section 245I.04, subdivision 4
• Demonstrated experience working with BIPOC, immigrant, LGBTQ+, and other marginalized communities.
• Knowledge of Twin Cities behavioral health, housing, and social service systems.
• Strong communication, organization, and collaboration skills.
• Valid driver's license with clean driving record and ability to operate a company van for outreach.

Responsibilities

• Lead follow-up engagement and provide short-term light case management services to recipients referred by the Responder Team.
• Use trauma-informed and harm reduction approaches to build trust, stabilize situations, and connect riders to immediate and long-term care.
• Accompany riders to appointments, shelters, or treatment programs as needed.
• Collaborate with county social services, shelters, hospitals, and behavioral health partners to coordinate care.
• Document all outreach activities, service plans, and referrals in accordance with HIPAA and Council reporting requirements.
• Participate in weekly clinical supervision meetings, case conferences, and supervision sessions.
• Support the Responder Team in the field when needed and provide logistical support.
• Educate riders and community stakeholders about resources, rights, and pathways to stability.
