Job summary

The role involves home visits, telephonic contacts and in-person coordination, with travel up to 40%. Requires state RN license, active EMR/HIPAA knowledge and strong communication skills.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent combination of relevant education and experience.
• RN license active and unrestricted in state of practice.
• Valid and unrestricted driver's license and reliable transportation for job-related travel.
• Understanding of EMR and HIPAA.
• Knowledge of community resources.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines qualification for care management.
• Develops and implements care coordination plan with member, caregiver, physician and other health professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and updates goals.
• Maintains ongoing member caseload and performs regular outreach.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides coordination assistance.
