Qualifications

• Minimum 2 years experience in health care, preferably in care management.
• RN with active, unrestricted license.
• Valid driver’s license and reliable transportation for travel.
• Knowledge of EMR and HIPAA.
• Ability to work independently and manage multiple priorities.
• Strong communication and problem-solving skills.
• Proficiency with Microsoft Office and online portals.
• Travel 25–40% locally.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines who may qualify for care management based on clinical judgment and triggers identified in assessments.
• Develops and implements care coordination plan with member, caregiver, physician and other health care professionals to address member needs and goals.
• Conducts telephonic, face-to-face or home visits as required.
• Performs ongoing monitoring of care plan to evaluate effectiveness and document interventions and goal achievement.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services for members including behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team (ICT) meetings and collaboration.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Assesses barriers to care and provides coordination to address concerns.
• May provide consultation, resources and recommendations to peers.
• Care manager RNs may handle complex member cases and medication regimens.
