Qualifications

• At least 2 years experience in health care, care management preferred.
• Active RN license in state of practice.
• Valid driver license and reliable auto insurance for job travel.
• Understanding of EMR systems and HIPAA requirements.
• Strong communication and time-management skills.
• Ability to work independently with minimal supervision.
• Proactive, detail-oriented, and able to manage multiple projects.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with member and care team.
• Conduct telephonic, face-to-face, or home visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration among care teams.
• Provide education and motivation using motivational interviewing.
• Identify barriers to care and coordinate resources to address them.
• Potentially provide consultation to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
• Travel 25-40% as required by state/contractual needs.
